Inpatient Palliative Care at UM Shore Regional Health

University of Maryland Shore Regional Health’s Palliative Care program is offered by a team of experts – physicians, nurse practitioners, nurses, pharmacists, social workers, and chaplains. The team works with each patient and his or her family to optimize health and comfort by:

  • assisting in the patient’s treatment and illness management preferences,

  • providing the needed education to manage symptoms, and 

  • planning for any care needs outside of the hospital.

Inpatient palliative care is offered at:

UM Shore Medical Centers (SMC) at Chestertown, Dorchester and Easton,and The Requard Rehabilitation Center for Acute Rehabilitation  in UM SMC at Easton- Patients may request or be recommended for an assessment for a Palliative Care consult through their attending physician or nurse practitioner. The hospitals also offer designated Palliative Care suites in which patients who are at end- of-life can stay in a home-like atmosphere while undergoing treatments to control symptoms and making decisions regarding care prior to transitioning home or to another facility.

UM Shore Nursing and Rehabilitation Center at Chestertown - Patients may request or be recommended for an assessment for a Palliative Care consult through their attending physician or nurse practitioner. The consult is provided face- to- face or via telemedicine, a technology that enables specialty care providers to consult “in real time” with patients and their loved ones from a remote location. This is accomplished via live video conferencing, image capturing, electronic transmission of medical information and remote examination tools. A member of the Palliative Care team is present with the patient and assists the provider who is conducting the examination via telemedicine technology. Subsequently, the provider discusses (over video conference) the patient’s care needs and the palliative plan of care. Telemedicine allows Palliative Care providers to incorporate not only the patient, but his or her family members and other members of the health care team to discuss the patient's care needs and palliative plan of care.
