Home Health Care

University of Maryland Shore Regional Health provides home-based care through two home health agencies, UM Chester River Home Care (serving residents of Kent and Queen Anne’s counties) and UM Shore Home Care (serving residents of Caroline, Dorchester and Talbot Counties), and also through Shore Wellness Partners, a community-based care program designed for clients in underserved communities who meet certain eligibility criteria. Our goal is to help each patient maintain optimal health and independence in his or her home setting, whether a family residence, a group home, or a residential care facility. We also strive to:

  • Educate patients and their families to properly use equipment at home

  • Ensure compliance with medication and rehabilitation regime

  • Assist hospitals and physicians in decreasing patient hospital length of stay and readmissions
  • Increase safety awareness  
  • Decrease fall risk

Working with each patient’s primary health care provider, UM Chester River Home Care (UM CRHC) and UM Shore Home Care (UM SHC) develop an individualized program of care to meet the patient’s needs and encourage his or her progress toward optimal health. Each home care client’s care plan includes:

  • A list of needed services

  • The professionals needed to provide these services

  • How often services are needed

  • Equipment needed

  • Food/nutrition guidelines

  • Anticipated results based on pre-set timelines

UM CRHC and UM SHC staff members maintain contact with all clients regarding their progress, and also review and update all care plans every 60 days or less.

Skilled nursing services provided by UM CRHC and UM SHC include the following:

  • Catheter care
  • CHF  care
  • COPD  care
  • Diabetes care and education
  • Diet instruction and nutritional support
  • Dressing (bandage) changes
  • Colostomy/Ileostomy
  • Injections
  • Intravenous therapy
  • Medication monitoring
  • Orthopedic care
  • Pain management
  • Patient and family education
  • Patient assessment and monitoring
  • Rehabilitation
  • Stroke care
  • Terminal illness care
  • Wound care

Physical Therapists

Our Physical Therapy Services restore balance, range of motion, strength and endurance, and prosthetics. Physical therapy helps patients improve their ability to walk or get up and down. Therapists also determine equipment needs in the home and help reduce patient's risk for falling.

Occupational Therapists

Our Occupational Therapy Services help patients to improve in daily tasks such as bathing, dressing and basic household activities. Therapists may recommend adaptations to make the home safer and establish a daily routine for important tasks.

Speech and Language Pathologists

Our Speech Language Pathology Services help patients and their families with memory problems, improving the ability to communicate, and restoring patient's ability to swallow safely.

Medical Social Workers

Our Medical Social Workers provide home care services such as social and emotional counseling, and help in locating community resources to help with financial concerns.

Home Health Aides

Home Health Aide services assist the patient with personal care such as bathing, toileting, dressing and also provide assistance with ambulation.
