Megan Pulleyn Coumadin Clinic
The Megan Pulleyn Coumadin Clinic is part of the Clinical Pharmacy Services provided by the Shore Medical Center at Chestertown. The clinic was established in January of 2011 and is run by the pharmacy department staff. Patients are referred to our clinic and seen primarily face to face by appointment. During a typical visit a capillary (finger stick) PT/INR is obtained and evaluated, and a patient’s warfarin therapy is specifically tailored to meet established guidelines. Detailed patient instructions, education and counseling are provided. The clinic also follows patients who are being cared for by various Home Health Nursing agencies, local Care Homes, as well as patients who have access to a home PT/INR device. Counseling and management for these patients is handled via the telephone.
Various medical conditions require inpatient and outpatient management of anticoagulation therapy with warfarin, Lovenox® (enoxaparin), Heparin, Arixtra® (fondaparinux) and the newer agents such as Xarelto®( rivaroxaban) Eliquis®(apixaban), Savasa® (endoxaban) and Pradaxa®(dabigatran). The Megan Pulleyn Coumadin clinic primarily focuses on Coumadin® (warfarin) management.
Your physician or Licensed Individual Practitioner (LIP) can refer you to our services where you will be followed by licensed clinical pharmacists who are experts in anticoagulation management.
A clinical pharmacist is available 7 days a week to answer questions 8am- 4:30pm and can be reached by calling the number below. If after usual hours and you need to speak to someone more urgently contact the main number for the hospital and have the clinical pharmacist on call paged.
